Participants learn about their mental illness and how to manage its impact on their functioning, create crisis plans, and work one on one with a mental health specialist on a weekly basis.
TSC (Targeted Service Coordination or Case Management) is a program designed to link participants who have a history of higher cost medical services (i.e., Psychiatric hospitalizations, ER visits, incarceration) to community resources such as housing, health care, employment assistance, financial resources, and transportation. This program is designed to prevent participants from facing homelessness, incarceration, or hospitalization.
- PSR
(Psycho-Social
Rehabilitation) is a program designed to help participants with
severe and persistant mental illnesses learn skills
necessary to successfully manage their mental illness and live more
independently within the community. It teaches clients how to make their
own healthy life decisions, and remain stable within the community.
Community-Based Services are designed to both teach people skills,
to increase independence and link participants to needed resources
in the community.
